ADMISSION
To all our valued clients who wish to enrol at the Provincial Training Center-Pangasinan (PTC), the following downloadable forms are available for you to fill up. Send thru our e-mail address or print the forms and submit them to the Provinical Training Center. Likewise, please prepare all the requirements needed for enrollment.
If you have other questions we are very much available to answer them.

REQUIREMENTS FOR TRAINING:
- 2 pcs. 1x1 ID Picture
- Birth Certificate from NSO or Xerox
copy of Passport
- High School Card/Diploma or OTR
- Medical Certificate
(Physically Fit for Training)
- Police or NBI Clearance
- Barangay Clearance
QUALIFICATIONS FOR TRAINING:
- at least 18 years old
- High School Graduate
